I like Mind The Gap, but it sort of implies you have to avoid falling inside the gap, and in this game, it's already too late for that. How about just "Gap"?
Go with something actually unique like OMG i just fell of a cliff!!! The Inevitable Outcome of a Frivolous Exercise in Free-fall, or It's Dark in this Tunnel... Is it Always this Dark?" ... not that those are that good but something like that is way more interesting and more likely to get your game noticed. The app store is about style first and foremost.

"Death Drop" "Drop Out" (or just "Dropout") "Epic Fall" I'm partial to that last one, nice little play on words with "epic fail".
I like Freefall Frenzy (like the way it sounds), Void (or possibly "The Void") and Epic Fall (nice wordplay, but not quite sure if it has a nice enough ring to it).
